Though I don't recall the details leading to the 2 frame gap between subtitles, I believe most of the recommendations in the document are due to issues with some equipment based on ISDCF tests. In designing the USL/QSC closed captioning equipment, I had to deal with zero frame gaps between captions....

Isn't just creating a DCP with two CPLs, one with subtitles and one without? Projectionist selects the correct one at playback time

In the SMPTE tests, the main picture had a grid with lines every 5% (I think!). It was pretty easy to tell where the subtitle landed.

I agree with your test results. The big difference from the tests I analyzed was that the text was vertical. Most systems did vertical positioning based on the bounding box. Thanks for the comments! The SMTPE group was working on improving the rendering of Japanese subtitles. The vertical position tests were done using vertical Japanese text. Yes, a mess, especially for vertical text. I think all that can be done right now is to define a safe area for subtitles to ensure they do not go off the screen. SMPTE ST 428-7 says to convert to italic by skewing the string to the right (I think it is 7 degrees). In tests, most, if not all, systems skewed the string about the horizontal baseline. Ascenders will move to the right and descenders will move to the left making the string slightly longer.

The Valign and Vposition values for the Text elements look correct, but as pointed out in the ISDCF document, some systems require the top line of text to be the first Text element. Here it is the bottom. That complies with SMPTE ST 428-7, but does not work properly on this system.
